Why this page exists
Mushroom coffee is sold as food, but the extracts in it are the same ones sold as supplements, and several have documented interactions with common medicines. The most careful public summaries are Memorial Sloan Kettering's herb monographs, which separate case reports from laboratory findings. The table condenses what they report for the mushrooms found in coffee.

Three medication groups to take seriously
Blood thinners and antiplatelets. Four of the mushrooms above — reishi, cordyceps, chaga, maitake — push in the same direction: less clotting. Individually the signals range from laboratory findings to single case reports. Together, in a blend, on top of warfarin, apixaban, clopidogrel or daily aspirin, that is a conversation to have with whoever manages your anticoagulation before you make the coffee a habit. The maitake–warfarin INR report is the most concrete example.
Glucose-lowering drugs. Maitake, cordyceps and chaga each show glucose-lowering effects, mostly in laboratory settings. If you take insulin, sulfonylureas or other hypoglycemics, the risk is additive lowering. Tell your prescriber and watch your readings when you start.
Kidney disease. Chaga is the specific concern. It is high in oxalate, and MSKCC documents kidney injury in adults taking it daily at supplement doses for months. A coffee serving is a far smaller amount, but if you have reduced kidney function, a history of stones, or take drugs cleared by the kidney, a chaga-free blend is the simple fix.
Surgery
There is no mushroom-specific washout data. What exists is general guidance: the American Society of Anesthesiologists asks patients to disclose every supplement before surgery because some raise bleeding risk, and the mushrooms above qualify. Tell your surgical and anaesthesia team what you drink and how much, and let them set the stop date. Do not decide on your own that a coffee doesn't count.
Does the small dose make it safe?
Smaller, not safe. Every case report above involved supplement-level doses, usually grams per day for months, and a coffee serving delivers a fraction of that. That genuinely lowers the probability of an interaction. It does not change the direction, and blends stack several mushrooms with overlapping effects. Frequently asked questions
Can I drink mushroom coffee on blood thinners?
Ask your prescriber first. Memorial Sloan Kettering reports that reishi can increase bleeding risk, that maitake has interacted with warfarin to raise INR, that cordyceps inhibits platelet aggregation in laboratory studies with one case of excessive bleeding after a dental procedure, and that chaga showed antiplatelet effects in animal studies. Coffee doses are small, but the direction of effect is consistent.
Does mushroom coffee affect blood sugar or diabetes medication?
It might, in the direction of lowering glucose. MSKCC notes maitake may lower blood glucose and increase the effect of hypoglycemic drugs in type 2 diabetes, and that cordyceps and chaga showed additive glucose-lowering effects in laboratory studies of unknown clinical relevance. If you take insulin or another glucose-lowering medicine, tell your prescriber and watch your readings.
Is chaga safe if I have kidney disease?
It is the mushroom with the clearest reason for caution. MSKCC documents oxalate nephropathy in two adults taking chaga powder daily for months, and end-stage renal disease in one person after long-term use. A coffee serving is far smaller than those doses, but if you have reduced kidney function or kidney stones, choose a chaga-free blend and tell your nephrologist.
Should I stop mushroom coffee before surgery?
Tell your surgical and anaesthesia team what you take and how much, including the coffee. The American Society of Anesthesiologists advises disclosing all supplements because some raise bleeding risk, and reishi, cordyceps, chaga and maitake all have antiplatelet or anticoagulant signals. Your team will tell you whether and when to stop; there is no mushroom-specific washout data.
